The Role of Medicare in Long-Term Care & Assisted Living
One of the most misunderstood areas of Medicare is what it does and doesn’t cover when it comes to long-term care and assisted living. Many seniors assume Medicare will pay for nursing homes or in-home assistance indefinitely—but that’s not the case.
At Medicare of Florida, we help families plan ahead for future care needs, understand what Medicare will pay for, and explore options for protecting their savings. In this post, we’ll explain how Medicare handles long-term care, what alternatives exist, and how to be fully prepared.
📌 What Is Long-Term Care?
Long-term care refers to a range of services that help people with daily activities, including:
✔️ Bathing
✔️ Dressing
✔️ Eating
✔️ Using the bathroom
✔️ Moving around safely
💡 This can be provided in nursing homes, assisted living facilities, or at home with caregiver support.
🚫 What Medicare Does NOT Cover
Medicare is not designed to cover custodial long-term care, which is care that helps with daily living activities rather than medical treatment.
That means Medicare does not cover:
❌ Assisted living facility costs
❌ Long-term nursing home stays beyond short-term rehab
❌ Ongoing home health aide services (unless medically necessary)
❌ Personal care like help with dressing or bathing
💡 Many families are surprised to learn they must pay out of pocket or use other resources like Medicaid or long-term care insurance.
✅ What Medicare Does Cover
While it doesn’t cover custodial care, Medicare does pay for medically necessary care, such as:
🏥 Short-Term Skilled Nursing Facility (SNF) Care
After a qualifying hospital stay of at least 3 days, Medicare may cover:
✔️ Up to 20 days at $0 cost
✔️ Days 21–100: $204/day coinsurance (2025)
✔️ After day 100: No coverage
🩺 Home Health Care
Medicare may pay for short-term skilled nursing care, therapy, or health aides if:
✔️ Ordered by a doctor
✔️ You are homebound
✔️ Care is intermittent and medically necessary
🏠 Hospice Care
Medicare covers palliative and comfort care for individuals with a terminal illness and less than 6 months to live.
💡 Medicare covers almost all hospice-related costs, including nursing, medications, and grief support for the family.
🧾 Who Pays for Assisted Living or Extended Care?
If you or a loved one needs care beyond what Medicare covers, here are your options:
🔹 Medicaid – For low-income individuals; may cover nursing home care
🔹 Veterans Benefits – VA offers long-term care for qualifying veterans
🔹 Long-Term Care Insurance – Policies that cover extended care at home or in facilities
🔹 Out-of-Pocket – Private pay for assisted living or in-home aides
🔹 Life Insurance with Living Benefits – Some policies allow access to death benefits while alive for care needs
